Beacon Mutual partners with independent agencies who are approved to bind workers’ compensation coverage in Rhode Island, Massachusetts, and in Connecticut. Our underwriters can assist you with any questions that you might have.

The following documents must be submitted to complete the agency approval process:

  1. Producer application. Download and complete the Producer Application, including the agency name and tax identification number (if you are not affiliated with an agency, complete the form with your name and social security number). In order to become approved with Beacon Mutual, the agency (or individual) must employ at least one person who holds and maintains a valid Rhode Island individual insurance producer license, which includes property/casualty lines of authority.
  2. W9 form. The latest version of the W9 form can be downloaded from the IRS website:  IRS.gov/pub/irs-pdf/fw9.pdf
  3.  Electronic Payments: If you would like to receive electronic payments for commission credits please provide your Financial Contact’s email address and direct telephone number (no extension). Click here to provide your Financial Contact information.  When your agency’s first commission payment is ready to be issued, the Financial Contact will receive an email from claimspay@oneinc.com. The Financial Contact will follow the steps to access the One Inc. payment system to set up electronic payments. To complete the One Inc. login, a security code will be sent to the Financial Contact at the option of either a text message (to mobile device) or automated telephone call to the Financial Contact’s direct line.
  4. BEACONNECT Registration: Download the BEACONNECT Registration form. The completion of this form allows Beacon Mutual to create your agency account in BEACONNECT, our secure online portal that allows you to access account and policy summaries, billing status, obtain loss runs, submit on-line New Business applications, as well as access to many other useful tools.
  5. Errors and Omissions (E & O) policy declaration page: Please include a copy of your most current policy with the application.
  6. NEW JERSEY and TEXAS ENTITIES ONLY:  Please provide a recent Standing Certificate (or Validation Certificate) or other documentation evidencing an active corporate charter.
